Submit Grant Application

Step 1: Enter all relevant information about yourself and Organization, then click "Next".
Step 2: When you are finished answering the questions, click the "Submit Completed Proposal" button to complete the process and submit all data to our office.
Step 2: If you are applying for the Connect Grant, you will need to include contact information for 2 references.
When entering phone numbers, do not use dashes, dots or spaces in between numbers.
